We accept most insurances, aside from First Care by itself, and Blue Cross Blue Shield HMO (we do accept BCBS PPO).
Your insurance card and ID. If you do not have insurance, your initial payment for self pay will be $190 and $90 for follow up appointments.
Call our office number (940-322-5477) and we will transfer you to our refill voicemail line. These voicemails are checked daily.

Due to our high patient volume your provider will more than likely always be with a patient when you call during business hours. If you call and leave a message, your provider will return your call as soon as possible. Our office number is 940-322-5477.

Yes! If your appointment is on a Monday we will call and remind you on Friday, if your appointment falls Tuesday-Thursday we will call you the day before.
Sign consent to release information, the results take up to 48 hours to receive. If you request the records, $25 will be due upon receipt of the package. If requested by a physician or therapist there will be no charge.
All new clients should complete their consent forms and health history prior to the first appointment. If possible, please complete and before coming to the office. If this is not possible, bring the completed forms with you at your first appointment.
